When sharing your feedback through the Wellby kiosk located in your doctor's office, you help to make it easier for you and everyone else in health care to communicate. You also help to increase safety by reducing the likelihood of dangerous mistakes. In addition, when you are at the kiosk you can choose to share your feedback with your Care Coordination Team (your doctor and other caregivers who focus on keeping you healthy) so they can be more effective in their work. Remember that with Wellby your personal feedback is protected and remains confidential.

• You and your loved ones are an integral part of the care team. You work with your health care professionals to make important health-related decisions.
• You take responsibility for managing and monitoring your own care.

Patient-centered care also means you are able to participate in making decisions and to set personal health goals that seem practical and within your control. As you take greater charge of your health, Wellby supports you by:

a healthy diet, and eliminating bad habits like smoking
• Helping you be involved in care decisions, working with
your health care team to create and carry out agreed-upon treatment plans
• Sharing relevant information with your care providers and clearly expressing your wants and needs, and
