This versatile kiosk even lets you scan your Personal Goal Planner, prescriptions, referrals or other materials so you can review them later via through Wellby.com.

Use Wellby.com to manage your health. During your doctor’s visit, you may have set personal health goals – perhaps to become more physically active, to manage your meals better, or take your medications more regularly. It’s now up to you to meet these goals. You can use your Personal Goal Planner to track your progress. But you can take advantage of a wealth of personal health resources on Wellby.com.

The riches in myPersonal Health Locker. On Wellby.com you can create an online Health Record – called myPersonal Health Locker – and store information about your medical condition, tests and personal health goals. What’s more, you can view your responses to the kiosk quality questions and items you scanned into the kiosk. And the next time you visit your doctor and use the Wellby kiosk, you can swipe any ID card to quickly connect with myPersonal Health Locker and store information.
